Job summary

The Registered Nurse (RN) I provides skilled nursing care in a patient's home, educates families, monitors patient health, and manages care coordination, ensuring legal compliance and adherence to scope of practice. The role involves performing assessments, administering treatments, documenting thoroughly, and collaborating with multidisciplinary teams.

Responsibilities

Provide skilled nursing care in patients’ homes, assess and monitor their conditions, administer prescribed medications, and develop or adjust individualized plans of care. Coordinate multidisciplinary services, educate patients and families, supervise aides and LPNs, and complete required clinical documentation and on-call duties.

Qualifications

Requires a current Registered Nurse license in the state of employment and at least one year of RN experience in an acute care setting; home care experience is desired. Candidates must have a valid driver’s license and auto insurance, along with strong communication, clinical judgment, organization, and patient-care skills.
