Job summary

This role involves managing care for high-risk, chronically ill patients, supporting interdisciplinary teams, conducting patient assessments, facilitating care plans, and communicating with patients and families in a home or clinical setting, primarily in palliative and hospice care.

Responsibilities

The Care Management RN supports the interdisciplinary team by facilitating care plans and ensuring treatments align with patient values and quality-of-life wishes. They conduct face-to-face assessments, manage caseloads, and lead sensitive end-of-life discussions with patients and families.

Qualifications

Candidates must have an active Ohio RN license and at least 5 years of clinical or case management experience in a hospital, home health, or hospice setting. Proficiency in computer software and the ability to travel throughout the service area are required.
