Job summary

Community Care seeks a registered nurse for case management of elderly and disabled adults in community settings. The role involves conducting assessments, developing care plans, and coordinating services to support independent living. Full-time position based in West Bend serving Washington and Dodge Counties.

Responsibilities

Conduct initial and ongoing nursing assessments, make home visits and assessments, and consult with families and other supports. Develop and maintain individualized care plans, track member outcomes, and follow up on services to ensure their quality, coordination, and effectiveness.

Qualifications

An associate degree in nursing and a Wisconsin RN license are required; a bachelor's degree is preferred. Candidates need eligibility for long-term care functional screener certification, a Wisconsin driver's license, and two years of recent nursing experience with older adults or people with physical or intellectual disabilities; two years of community nursing experience is preferred.
