Job summary

The Community Health Worker (CHW) supports members with complex medical and social needs by managing their own health and wellbeing. They conduct assessments, develop care plans, build rapport, coach to minimize health risks, accompany patients to appointments, connect members to programs, document encounters, and engage eligible patients into the Akido ECM Program. Requires high school diploma, bilingual in English and Spanish preferred, 2+ years experience with CalAIM populations, and ability to work outdoors and independently. Travel required. Compensation is $22-$28 per hour.

Responsibilities

The Community Health Worker manages a caseload of patients by developing care plans, providing health education, and facilitating behavioral changes. They also accompany members to medical and social service appointments while documenting all encounters in case management software.

Qualifications

Candidates must have a high school diploma and at least two years of experience working with vulnerable populations such as those experiencing homelessness or complex medical needs.
