Job summary

The Community Health Worker (CHW) supports members with complex medical and social needs by managing their own health and wellbeing. They conduct assessments, develop care plans, build rapport, coach to minimize health risks, accompany patients to appointments, connect members to programs, document encounters, and engage eligible patients into the Akido ECM Program. Requires high school diploma, bilingual in English and Spanish preferred, 2+ years experience with CalAIM populations, and ability to work outdoors and independently. Travel required. Compensation is $22-$28 per hour.

Responsibilities

The Community Health Worker will manage a caseload of patients with complex medical and social needs by developing care plans and providing behavioral coaching. They will also facilitate access to medical and social services while documenting all patient encounters in the case management system.

Qualifications

Candidates must have a high school diploma or equivalent and at least two years of experience working with CalAIM populations. A valid driver's license, reliable transportation, and the ability to work in dynamic, outdoor environments are required.
