Job summary

Triage Navigators at Heartland Family Service will be the first point of contact for clients, responsible for screening, assessing, and assisting clients with crisis stabilization and resource linkage in a trauma-informed, supportive environment during overnight shifts.

Responsibilities

The Triage Navigator serves as the first point of contact for consumers, conducting screenings and assessments to determine appropriate levels of care. They are responsible for creating stabilization plans, providing crisis intervention, and facilitating life-skill building activities for clients.

Qualifications

Candidates must have at least two years of direct care experience in the human services field or a bachelor's degree in psychology, sociology, or a related field. The role requires the ability to work independently and as part of a team in a fast-paced, trauma-informed environment.
