Job summary

Coordinates patient care in home setting, performs assessments, follows individualized care plans, provides education on health issues including end-of-life, chronic disease, and pain management, and performs skilled treatments under supervision.

Responsibilities

Coordinates total plan of care with health care professionals and manages individualized, goal-directed nursing care for patients in a home setting. Performs comprehensive patient assessments and provides education on chronic disease management, palliative care, and symptom control.

Qualifications

Requires an Associate’s Degree or Specialized Diploma in nursing with at least 1-2 years of recent direct care experience. Must hold a valid Pennsylvania Registered Nurse license, Basic Life Support certification, and a driver's license.
