Job summary

Provide comprehensive nursing care to individuals with mental health and co-occurring conditions in community settings, including medication management, assessments, health education, and care coordination as part of a multidisciplinary team.

Responsibilities

Provide community-based nursing care to people with severe and persistent mental illness and co-occurring conditions, including assessments, medication management, treatments, health education, and crisis support. Coordinate care with participants, families, healthcare providers, and an interdisciplinary team while maintaining accurate documentation and compliance with applicable safety and privacy requirements.

Qualifications

Applicants must hold a current Delaware RN license, graduate from an accredited nursing school, have CPR certification, and meet background, security clearance, driver’s license, vehicle, and travel requirements. A BSN and at least one year of clinical nursing experience are preferred, particularly experience in behavioral health, psychiatric nursing, or community health.
