Job summary

Provide hospice nursing care as a trusted case manager, guiding patients and families through care decisions, assessments, and symptom management while collaborating with an interdisciplinary team.

Responsibilities

The Hospice RN Case Manager will conduct comprehensive patient assessments and develop individualized care plans to provide skilled nursing and comfort. They will also educate families, manage symptoms, and collaborate with an interdisciplinary team to ensure high-quality end-of-life care.

Qualifications

Candidates must hold a current RN license in the State of Colorado and possess at least two years of nursing experience, preferably in hospice or home health. A valid driver's license and a reliable, insured vehicle are also required for home visits.
