Responsibilities

The Hospice RN Care Manager provides compassionate, patient-centered nursing care in homes and facilities while developing and adjusting individualized plans of care. They coordinate with an interdisciplinary team, communicate patient needs to physicians, and provide education and support to patients and their families.

Qualifications

Candidates must be graduates of an accredited nursing program with a current Ohio RN license and CPR certification. A valid driver's license, reliable transportation, and the ability to travel throughout the assigned territory are required.
