Job summary

Become a part of our caring community by providing personalized home health nursing care, including medication administration, wound care, patient assessments, care plan development, patient education, and overseeing aides, supporting recovery and independence.

Responsibilities

The Registered Nurse Case Manager provides personalized, one-on-one skilled nursing care to patients in their homes, including assessments, wound care, and medication administration. They also develop individualized care plans, educate families, and coordinate with a multidisciplinary team to support patient recovery.

Qualifications

acute experience care planning ER ICU med surg medication administration nursing patient assessment patient monitoring wound care

Candidates must have a current and unrestricted Registered Nurse license, CPR certification, and experience in med-surg, ICU, ER, or acute care settings. A valid driver's license, reliable transportation, and auto insurance are also required for this field-based role.
