Job summary

The Integrated Case Manager coordinates transitional care for children by assessing their needs, developing care plans, and facilitating communication among providers, families, and community resources, ensuring safe discharge and continuum of care.

Responsibilities

Assess patients’ clinical, physical, and behavioral needs, then coordinate individualized care plans with families, providers, hospitals, and community services. Support safe and efficient transitions and discharges, monitor level-of-care and length-of-stay concerns, and arrange access to appropriate aftercare.

An associate degree in nursing and a Wisconsin Registered Nurse license are required; a bachelor’s degree in nursing is preferred.
