Seeking a Primary Care Physician to provide comprehensive, value-based care for seniors in Richmond, Virginia. The role involves managing a panel of up to 450 patients, leading a care team, and engaging in clinical assessments, health promotion, and care coordination within a Medicare Advantage model. Candidates should have MD or DO credentials in relevant specialties, active Virginia licensure or ability to obtain it, and proficiency in English and Microsoft Office.

Responsibilities

The physician will provide comprehensive primary care to older adults in a value-based model, leading a dedicated care team to manage patient health outcomes. Responsibilities include diagnosing and treating acute and chronic illnesses, coordinating care across settings, and actively participating in center management.

Qualifications
