Job summary

Provide compassionate care and support to hospice patients in their homes, monitoring patient condition, educating caregivers, and collaborating with healthcare team to deliver personalized care.

Responsibilities

The Hospice Aide provides skilled nursing care and monitors patient conditions according to the individualized Plan of Care. They collaborate with the RN Case Manager to address patient needs and educate caregivers on self-care techniques to support independence.

Qualifications

Basic Life Support (BLS) Communication Empathy Medication Administration Observation Patient Care

Candidates must have a High School Diploma or GED and at least one year of experience as a Hospice Aide or Nursing Assistant. A valid driver's license and completion of a state-established Hospice Aide training program are also required.
