Job summary

A Licensed Practical Nurse (LPN) provides direct nursing care to older adults and individuals with disabilities in an Adult Day Health setting. Responsibilities include administering medications, monitoring health, and collaborating with the care team to implement individualized care plans.

Responsibilities

The LPN will provide direct nursing care and clinical support by implementing individualized care plans and administering medications. They will also collaborate with the interdisciplinary team to monitor participant health and maintain accurate clinical documentation.

Qualifications

Candidates must hold a current, unrestricted Massachusetts Licensed Practical Nurse (LPN) license. Preferred qualifications include 1–2 years of nursing experience with older adults or individuals with chronic health conditions.
