Qualifications

Care Coordination Case Management Community Resources Medical Services Nursing Patient Education Program Development

Requires a graduate from an accredited school of nursing with 4–6 years of related clinical or case management experience. A background check by the Texas Department of Family & Protective Services is mandatory.
