Job summary

The Care Coordinator supports youth and families in health and outcome goals through community-based service linkage, care coordination, scheduling, documentation, and follow-up, mainly serving as a primary contact in the OhioRISE plan.

Responsibilities

The Care Coordinator assesses the needs of youth and families to provide service and resource linkage within the OhioRISE plan. They serve as the primary point of contact for support providers, managing documentation, scheduling meetings, and tracking health outcomes.

Qualifications

Candidates must hold a Bachelor's degree in a related field or have at least three years of experience in behavioral health or human services. A valid Ohio driver's license, car insurance, and the ability to complete required training certifications are also mandatory.
