Job summary

The VNA of the Treasure Coast seeks a full-time Licensed Practical Nurse (LPN) to provide skilled nursing care in home health, following assessments and care plans developed by RNs. Responsibilities include medication administration, patient observation, education, documentation, and collaboration with healthcare team, all within a community service setting.

Responsibilities

The LPN will provide skilled nursing care to patients in their homes, including administering medications and treatments as prescribed. They will also monitor patient conditions, maintain clinical records, and collaborate with the care team to ensure adherence to established care plans.

Qualifications

Candidates must hold a current LPN license in the state of Florida and have previous home health experience. IV certification and lab skills are highly preferred, and the role requires passing a background screening.
