Job summary

Provide compassionate hospice nursing care, including assessment, symptom management, and patient education, leading interdisciplinary teams in home settings.

Responsibilities

The Primary RN manages an assigned caseload of hospice patients and leads the interdisciplinary team to provide compassionate, relationship-based care. They are responsible for symptom management, patient assessment, and educating caregivers on care goals.

Qualifications

Candidates must hold a current Colorado RN license and CPR certification, with 1-2 years of nursing experience preferred. A valid driver's license and reliable transportation are required for travel to patient locations.
