Job summary

The RN Care Manager at Akin Care assesses patients, develops care plans, mentors caregivers, and ensures compliance and quality in a home care setting, working collaboratively with families and healthcare professionals to provide personalized and safe senior care.

Responsibilities

The RN Care Manager will conduct in-home assessments, develop individualized care plans, and supervise caregivers to ensure high-quality, compliant care. They will also collaborate with healthcare professionals and families to advocate for client needs and maintain accurate medical documentation.

Qualifications

Candidates must hold an active Registered Nurse license in New Jersey and possess either a bachelor's degree in nursing or at least two years of geriatric/home-care experience. Additionally, a valid driver's license and CPR/First Aid certification are required.
