Job summary

A Registered Nurse (RN) Case Manager provides comprehensive patient assessments, care planning, and coordination in hospice settings, ensuring compliance with laws and fostering collaboration among healthcare professionals.

Responsibilities

Assess patients’ physical, psychosocial, environmental, cultural, and spiritual needs, and implement and revise individualized plans of care. Coordinate care with patients, caregivers, physicians, and interdisciplinary teams; provide assigned on-call services, submit documentation promptly, and ensure regulatory compliance.

Qualifications

Applicants must graduate from an accredited diploma, associate, or baccalaureate nursing program and hold a current state Registered Nurse license. A current driver’s license, reliable transportation, and valid auto liability insurance are required; two years of RN experience in a healthcare or clinical setting is preferred.
