Job summary

Provide nursing care to rehabilitation patients by assessing needs, planning, executing interventions, and educating patients and families, working collaboratively with a healthcare team.

Responsibilities

Assess patients, identify care needs, initiate appropriate nursing interventions, and provide age- and condition-appropriate care, including education for patients and families during rehabilitation and before discharge. Collaborate with the interdisciplinary team, maintain timely and accurate medical documentation and care plans, and participate in quality and performance improvement activities.

Qualifications

Critical Thinking Decisive Judgement Interdisciplinary Care Planning Patient and Family Teaching Patient Assessment

An active Wisconsin or compact-state LPN license and an associate degree are required; the license must be updated to the new primary state of residence before employment if relocating. AHA Basic Life Support certification is required within 30 days of hire, and one year of prior experience is preferred; candidates must also demonstrate critical thinking, sound judgment, and the ability to work with minimal supervision in a stressful environment.
