Job summary

The RN Care Coordinator in a hospital ambulatory setting manages patient care, conducts assessments, educates patients, coordinates follow-up, and collaborates with healthcare teams to ensure high-quality outpatient care.

Responsibilities

Provide direct nursing care and coordinate services for patients with complex and chronic conditions in an ambulatory pulmonary clinic, including assessment, treatment, education, and individualized care planning. Collaborate with patients, families, and interdisciplinary teams to coordinate referrals and follow-up, support safe transitions, document care, and contribute to quality and safety initiatives.
