Job summary

Licensed Practical Nurse providing patient care, assessments, education, and documentation in a rehabilitation hospital setting, working with interdisciplinary teams to improve patient outcomes.

Responsibilities

Assess patients, identify care needs, initiate appropriate nursing interventions, and provide age-appropriate care and education to patients and families during rehabilitation and before discharge. Collaborate with the interdisciplinary team, maintain medical necessity documentation and care plans, and participate in quality and performance improvement activities.

An active Wisconsin or compact-state LPN license and an associate degree are required; one year of prior experience is preferred.
