Responsibilities

The RN Case Manager is responsible for developing and implementing individualized plans of care for hospice patients while coordinating with physicians and interdisciplinary teams. They perform initial assessments, provide direct nursing care, and educate caregivers on end-of-life issues and treatment plans.

Qualifications
