Job summary

Licensed Practical Nurse (LPN) providing patient-centered nursing care in home health settings, monitoring patient conditions, administering medications, documenting visits, and collaborating with healthcare teams.

Responsibilities

Provide skilled nursing care in patients' homes, monitor their health, administer prescribed medications and treatments, and document visits in Homecare Homebase. Educate patients and families, report changes in condition to the supervising RN and physician, and collaborate with the interdisciplinary team while following applicable regulations.

Qualifications

Applicants must hold an active Michigan LPN license in good standing, have graduated from an accredited practical nursing program, and maintain current BLS/CPR certification. They must also have a valid driver's license, reliable transportation, current auto insurance, strong clinical, communication, and documentation skills, and the ability to work independently; home health, hospice, HCHB/EMR, and wound care experience are preferred.
