Job summary

A Clinical Care Navigator provides clinical support, psychoeducation, risk assessment, crisis intervention, care planning, and coordination for mental health members, mainly via outbound calls, requiring licensed mental health professionals with at least 2 years of experience.

Responsibilities

Provide in-the-moment clinical support, crisis intervention, and psychoeducation to members via an outbound phone team. Coordinate care plans and collaborate with multi-disciplinary teams to ensure a seamless member experience.

Qualifications

Requires an independent clinical license such as LCSW, LMFT, LPC, or LICSW. Candidates must have at least 2 years of direct clinical experience and expertise in crisis assessment and safety planning.
